Addindexdialogdoesnotopenonchoosingindexfromcreatetablepage

Steps:
1. Open create table interface page
2. Select any option from Index dropdown.
3. Add Index dialog does not appear (Stuck at "Loading...")

Edit 1:

I have configured 2 servers. On server 1 it works fine but when I am logged in to server 2 it does not work. So, seems like it is passing wrong server number or not at all passing server number.
